Planning an Event at First & Calvary

First & Calvary is a busy congregation; therefore it is important that all events are scheduled well in advance and childcare and media needs are secured. Those planning the event are responsible to complete the necessary forms and submit them to the Church office. If there are changes (room, time, location, etc.) after the forms are submitted, or if the event is cancelled, you must complete a revised form.

You can pick up the forms you need at the Church office during normal business hours, Monday through Friday, from 8:30 AM to 4:30 PM or download them below.

No event is considered scheduled until an Application for Building and
  Facility Use Form has been submitted and approved by the Office staff.
